I'm pretty sure this doesn't interact with Wild Shape at all. There is no generic Wand of Hybrid Form item. Each Wand will contain one specific polymorph spell. All polymorph spells list a variety of options you can choose from, and that's all the Wand lets you combine, two options from those listed in the polymorph spell contained in the wand.

O.O Wild shape is many things but I would NOT call it simple. It took me a long ass time to wrap my head around how all the wild druid pieces fit together.
